Sarasota, FL (WorkersCompensation.com) -- I am working with a man who fell while he was in Mexico on vacation with his family. He suffered a brain injury and is confused and agitated. As a result, he was airlifted to Miami. His wife, adult son and daughter are with him.
As you know, with a TBI, mentation and agitation are part of the diagnosis. Keeping him calm, allowing him to rest is the goal.
Another challenge is that the patient is from Oregon and wants to go back to his home. This will require him to be medically stable and possibly an air ambulance for transport.
As a Nurse Case Manager, my role is to set realistic expectations with the family and help them understand the process. As an independent case manager, I learned a long time ago, we can only do so much and we have to let the hospital staff do their job and let the process move forward.
Care Coordination and transition of care is part of the role as well as being a support to the patient, the family and the care team to work through challenges that come up.
