Okay, since you have a consent form. I would think the IC has a copy? Being if that is so, I would think such information would come up for them. being said, they then ask if your a relative, and wait your response to see if it match the consent information.
Now with me, I had to sign over Power of Attorney for any of this to even be thought of. I would think this can be different per State also. Reply's are intended solely for informational purposes.
