Copies of Doctors papers

I'm in California
My case was settled in 1998 in LA County with a cash payout no future medical

Due to my movement restrictions my Current employer is requesting that I provide them with a "Doctors" note outlining my disabilities , so they can accomadate me at work.
I would like to get ahold of the original doctors Final Report as this outlines my disability .
My lawer I used is no longer in business and I cant remember the doctors name , where can I look for this Information?
I tried searching but so far nothing.

The WCAB had the original legal file but I suspect it has been destroyed as they had a project to reduce their storage needs a few years back. you can try contacting your local WCAB office or the Insurance Carrier that handled your claim.
