Application for Adjudication of Claim

RE: Application for Adjudication of Claim
Chris: You do not have to file the application to be entitled to all of your medical reports. The IC is supposed to serve them to you if you have no attorney. In order to make sure this happens, send a simple letter requesting "service of all medical records or reports in accordance with CCR 10608, 10609, et seq."

This CCR requires the IC to serve you within 6 days of your request, and continue to serve you every report received within 6 days of receipt.

RE: Application for Adjudication of Claim
Hi Chris. When I received my MRI's from the Hospital, all it took was a phone call. Actually I used the phone to get most of my copies. My attorney sends me copies of every correspondence he does with anyone, along with their responses. But the phone actually proved the best for me in receiving my Medical files. Good Luck and I hope all goes well!Wink
