on your brouser type in "workers compensation form 20". I did a brief search and found 5 different states that use a form 20.

8-05, Micro laminectomy/disectomy. 10-05 lumbar fusion L5-S1. 2-07 exploritory surgery. 12-07 medical implant, Spinal Cord Stimulator. now receiving SSDI. Jesus died for our sins. Soilders died for our freedom.

The form 20 is used to calculate your "Average weekly wages" based on your earnings during the period of 52 weeks immediately before the date of the injury. This is what your weekly payments will be based on.

The statute and form can be found on this website. Go up the the State Info tab above and select your state. The statute for calculating average weekly wage is 42-1-40.

hi kate, thanks for the advice....it sounds like u know ur stuff..so the bottom line of the wage thing...a week beforre i became unable to work i was given a raise from 10.50 to 12.00 per hr..there fore...am i entitled to collect the difference.. i guess what im saying is if they calculate the way u say then sounds like im gettin screwed..as i would b makin 12.00 per hr if i were working

your benifits would be based on 51 weeks at 10.50 and 1 week at 12.00. After doing the math, take that number and divide by 52. that will be your weekly average.
