Workman's Comp.help In California
I am trying to file a workman's comp settlement claim for loss of hearing.I lost hearing in both ears over the 23years I worked for my employer.The problem is that I was resently fired.My ex employer won't fill out the employer portion of the workman's comp.form.He also refuses to give me the name of the workman's comp.Insurance company.How do I find out who the Insurer is? And once I do,Can I send the form to the Insurer without the employer portion filled out or signed,Since they refuse to do it? I don't know what else to do.

welcome cowboy---------others will be along to help you with answers.........i would think your state must have some recourse open to i/w in your situation.....................need research or links to find out who, what, where, all that stuff..................
most states have someone who serves as a patient/i/w advocate and who will help you..........might get the spelling wrong--it's late and been a long day but you will be looking for the ombudsman -----funny name good luck and let us kmow what you learn nurse83 "After all, tomorrow is another day." gey 2 wheels look at the top of this page and click on state infro
Someplace it will give you the phone number to your w/c offices .. call them and say I have been hurt BUT my employer wont give me the name of their insurance Worked well for me and got the w/c commission on their butts for not turning it in |

unfortunately california does not have online lookup of coverage information.
The employer is required to post such information on the bulletin board located in an easily and centally accessible place, You certainly can file directly with the carrier. at this point you can enlist the help of the information and assistance officer http://www.dir.ca.gov/dwc/IandA.html you might want to send a copy of the claim form to the employer return receipt requested and keep the receipt. there is a penalty for failure to respond to the claim form. |
