Sad ssd/adl can anyone advise?
I was told by adjudicator that my claim was close to a decision and due to the holidays I should hear something by the 1st. Monday I rec the ADL forms. I have read that a denial decision has usually already been made at this point and they ask for these forms to back up the reason for denial. Anyone know if that is true? Any advice as to filling the forms out to be truthfull but also to express with accuracy the effects of my problems regarding my adl's?

Can you explain "ADL FORMS"?? I see no reason for SSA to send out forms that need to be filled out in reference to a disability, if a decision has been made.

8-05, Micro laminectomy/disectomy. 10-05 lumbar fusion L5-S1. 2-07 exploritory surgery. 12-07 medical implant, Spinal Cord Stimulator. now receiving SSDI. Jesus died for our sins. Soilders died for our freedom.

ADL/activity of daily living. I am assuming that all who apply for ssd must fill this form out but I don't know at what point in a claim it is generally sent out.
The adjudicator told me that the case was close to a decision and I should here something before the 1st, then came the forms to fill out. She had to know the day I talked with her that she was sending them out. I feel this means a denial.

not necessarily a denial - could mean that they are updating their files because of the length of time from the hearing. Here in Oklahoma from the time of the hearing in front of the ALJ until the decision is now taking about 9 months on average I was told. They do expect your adl to change during that time some what - abilities to do some things etc.

I am aware of the forms. The abbreviation threw me off. I received the forms after I applied and again when I requested reconcideration. I do not think a decision has been made. The forms are to give them a better idea of how your condition affects your everyday living. Answere the questions as best you can and make copies. The copies will come in handy if you are sent the forms again.

When i went for my PIR rating the 7th of this month, that was one of the questions i was asked. How it effects my everyday living. Brushing my teeth, hair, etc. Even effects going to the bathroom ( not to get to explicit) but hard if dominant hand to wipe yourself. That is why i take my meds while waking up before i go to the bathroom as it is not so painful after it kicks in. These are very important in the rating, so be up front and honest and like me, do not be embarrassed to say what it REALLY effects.

carpal tunnel recurrence/ neuropathy / RSD.
1/29/07 injury date. Permanent. PIR settlement 8/4/08 10%
