First I highly doubt anyone is going to jail. $1,000.00 a day seems that it won't happen neither, for I never seen it yet. Penalty I would say yes, or could happen. Insurance Attorney doesn't pay you, the Insurance carrier does.

You may have to wait till this reaches the hearing date to see what happens.

you're probably correct; there are doctors who don't like patients who don't get well.
most states have provisions for changing doctors.
Your location is not in your profile so you'll have to check with your state work comp board for information on how to do that. Don't wait until you are MMI request a change before that happens.
Not all violations of work comp laws are criminal offenses; most just result in fines or penalties.
I don't know what state you are in but you can look up the penalty for comp law violations in your state.
(it wouldn't help you much if your adjuster was in jail unless they allowed them to work on your case there!)
