If P*S is defined as PTP foreseeing that your condition will not get worse or better within the next 12 months and ongoing treatment is optional, my question is:

If recent therapies and exercises contributed to a relapse or increase in symptoms and PTP was fully informed of recent events, and still plans to PS, may an addendum be requested in the WC chart if PTP selectively chose not to document this?

If addendums are allowed is there a deadline to request this before rights are lost?

don't know about use of an addendum.
either party can object to a ptp determination by requesting a QME.
