How long does it take to get approval from IC for a injection

I am in CA awaiting an injection appointment to be approved from the IC. My doctor appt. was Nov 15, 2011, and the doctor requested the procedure right away. My doctors office is still waiting for the ok. I have a 1 month window from when it is approved to get it. What is there time limit ?

The decision on an Request For Authorization submitted for prospective review must be made within five business days from first receipt of the request, unless additional reasonable medical information is needed to make the decision. In that case, the additional reasonable medical information must be requested by the fifth business day, then up to 14 calendar days from the date of receipt of the original Request For Authorization are allowed for making the decision on the Request For Authorization.
